Parental Choice Library Book Restriction Form
Parents may utilize this form to restrict titles their child may check out in the school library. Please complete the form and return it to your child's teacher or librarian. This form is only required when a parent is seeking to restrict access to materials in the media center.

Students K-2 can earn SSYRA Jr brag tags for reading these books at home. Please fill out the SSYRA Jr. letter and return it once you and your child have read all 15 books or have decided you are done reading. We will make your child’s brag tag necklace, once we have received the letter. Click on the SSYRA Jr. link to find out more information. Have fun reading!

Students in grades 3-8 can earn brag tags after reading any of the SSYRA Grades 3-5 and SSYRA Grades 6-8 books. There are brag tags for every SSYRA book and incentives to read 5, 10 , or all 15 books on the list! Students should visit to the Media Center before school or during their Media #1 resource to get their brag tags and rewards. Click on the SSYRA links for more information.

We want to foster a lifelong love of learning, while inspiring our students to be creative, innovative, critical thinkers through reading, research, and exploration.
